Complex Care Clinic (C3) Program Manager, Cameron Park, CA
The C3 Program Manager will be engaged in the day-to-day operations of C3 Program, which operates a Medically Assisted Treatment Program (MAT) and related activities within a primary care setting in a community health center. The C3 Program Manager oversees the MAT program components and staff as well as related efforts to comply with established guidelines and best practices to contain, reduce and educate on the prescribing of opioids and other pain medication within a primary care setting.
The Program Manager is the key operational member of the multi-disciplinary C3 Team consisting of clinical and behavioral health providers, case management, patient navigator and support staff. The Program Manager facilitates strategic planning, program development with ensuring the documentation of procedures and workflows, grant management with reporting, and collaboration with Center Leadership and key internal staff as well as community partners.
The C3 Manager is also a clinician on the Center’s primary care provider team. The C3 Manager will have a MAT patient panel and work clinical hours of up to twenty hours per week to the licensure and scope of practice the clinician holds.
The C3 Manager is a highly motivated individual with the experience and passion for engaging with staff, treating Center patients in a primary care setting, and working with our patient population in the medically assisted treatment program(s).
QUALIFICATIONS & EXPERIENCE:
- At least three (3) years direct service experience with adults who struggle with addiction and/or mental health issues, in an outpatient setting.
- Three (3) years management and program coordination experience in a health care setting.
EDUCATION & LICENSING REQUIREMENTS:
Candidates must be knowledgeable in current addiction medicine and dual diagnosis treatment modalities.
- Current unrestricted California State License without any limitations to practice in one of the following: Physician, Nurse Practitioner, Licensed Clinical Social Worker, Registered Nurse MSN, Marriage and Family Therapist, Licensed Psychologist or related advanced practice area.
- Strong program administrative management experience with appropriate Bachelor’s degree or higher and knowledge and experience in addiction medicine within an outpatient setting, will be considered in lieu of a clinical license.
